Chelsea 's first Premier League title success in five years sees Jose Mourinho's men automatically qualify for next year's Champions
League group stage.
Trailing eight points behind in second, last year's English champions Manchester City will also be joining them with third-placed Arsenal qualifying for Europe's elite for the 18th consecutive season.
Louis Van Gaal returned Manchester United to the Champions League in his first season in charge at Old Trafford but they will have to progress through the play off rounds after finishing in fourth spot.
The Europa League places are filled by Brendan Rodgers' Liverpool, who will enter the competition via the play offs, and Tottenham, who will begin their campaign in the third qualifying round.
One other contender for Europa League football next season is Aston Villa. The Villains could be handed a place in next year's competition should they beat Arsene Wenger's Gunners in the FA Cup final at the end of May.
At the other end of the table, QPR will play Championship football next season after ending the year with just 30 points.
The Hoops will be joined in the second tier by Burnley, who fell through the trapdoor despite a spirited effort. The final relegation spot goes to Hull City who are relegated after two seasons in the top flight.
